The team is always ready to help with your scheduling, insurance or anything else you may need during your time with us. They’ll take extra care to listen your concerns, answer your questions and thoroughly explain any condition and treatment options. What to bring to your appointment
- Picture ID (i.e. valid driver's license or passport)
- Insurance card
- Pharmacy number and location
- All current medications or a list with names of medications, prescriber’s name, strength and dosage (Important to know for checking drug interactions)
- Pertinent medical records in your possession
- Imaging reports (ultrasounds, CT scans, MRI, mammogram reports, bone density test reports)
- Operative notes, pathology reports, genetic test results for cancer screening, etc.
